Our chronic care program was created for residents that typical do not respond to other treatment programs. In this program the residents do not receive intensive treatment. They are considered both high functioning or lower functioning adults. This type of lifetime service was created for families to have an option in the event of their passing or inability to care for their loved ones. This program focuses on providing a stable and safe community for the residents to live.
Residents are provided:
• Leisure activities
• Weekly social outings
• Art and Crafts
• Daily physical activities to maintain physical and mental health
• Medication management
• Psychiatric services
• Individual Counseling (optional)
• Group Counseling
